Thank you for scheduling your appointment. Please be ON TIME for appointments, as they are back-to-back. These appointments will be held at the FAC Campus, located 5000 Sauk Trail, Richton Park. Enter through the MAIN ENTRANCE and Security will assist you.

Required Residency Documents for All Returning students

One of the following: Driver’s License, State ID or Other Photo ID

One of the following: Current Lease, Mortgage Statement, Deed or Tax Bill

Any (2) of the following: (dated within the last 60 days) Utility Bill(s)(1 Utility is Required), Phone Bill, Cable/Internet Bill, Car Registration/Village, Pay Stub, Home/Renters Insurance, Bank Statement

Required Guardian Documents for Returning Students Grades 10-12

  • Birth Certificate, Divorce/Custody/Guardianship Papers, /906 Placement Form

Required Medical 12th Grade Students Only!

  • Physical & Immunizations Records- (12th GRADE)

  • Seniors must submit proof of receiving the meningococcal vaccinationafter turning 16.This is a requirement by Illinois State Law for incoming seniors.
