RETURNING STUDENTS REQUIRED DOCUMENTS

Thank you for scheduling your appointment. These appointments will be held at the STEM campus, located  3600 W 203rd Street Olympia Fields. Enter through DOOR 1 and Security will assist you.

All Affidavits must be RENEWED Yearly!

Required Residency Documents for All Returning students

One of the following: Driver’s License, State ID or Other Photo ID

One of the following: Current Lease, Mortgage Statement, Deed or Tax Bill

Any (2) of the following: (dated within the last 60 days) Utility Bill(s)(1 Utility is Required), Phone Bill, Cable/Internet Bill, Car Registration/Village, Pay Stub, Home/Renters Insurance, Bank Statement

Required Guardian Documents for Returning Students Grades 10-12

• Birth Certificate, Divorce/Custody/Guardianship Papers, /906 Placement Form

Required Medical 12th Grade Students Only!

• Physical & Immunizations Records- (12th GRADE)
