Required Student Documents Incoming Freshman Class of 2029

Proof of Guardianship 

Birth Certificate,Divorce/Custody/Guardianship Papers,/906 Placement Form 

Test Scores

IEP/504/Special Ed Records (if applicable)

8th Grade Report Card & Diploma/Proof of Promotion

Physical & Immunizations Records (9th grade or more recent, out of state students must have IL physical)

Required Transfer Students Documents Grades 10-12

Proof of Guardianship = Birth Certificate,Divorce/Custody/Guardianship Papers,/906 Placement Form 

ISBE Student Good Standing Form (Illinois transfer students)

Withdrawal Papers

Test Scores

Unofficial Transcript

Required Residency Proofs for all students

*NOTE - if the parent/guardian is not the homeowner or leaseholder, then they need to additionally provide the following documentation:Notarized Affidavit of Residency. Driver’s License, State or Other Photo ID (with current address) required residency proof from the list above  (with current address)
