Thank you for scheduling your appointment. Please be on time for appointments, as they are back-to- back. These appointments will be held at the FAC Campus, located 5000 Sauk Trail, Richton Park. Enter through the MAIN ENTRANCE and Security will assist you.

Required Residency Proofs for All students

One of the following: Driver’s License, State ID or Other Photo ID

One of the following: Current Lease, Mortgage Statement, Deed or Tax Bill

Any (2) of the following: (dated within the last 60 days) Utility Bill(s)(1 Utility is Required), Phone Bill, Cable/Internet Bill, Car Registration/Village, Pay Stub, Home/Renters Insurance, Bank Statement

Required for Incoming Freshman Class of 2029

Proof of Guardianship 

• Birth Certificate, Divorce/Custody/Guardianship Papers, 906 Placement Form
• 8th Grade Report Card & Diploma/Proof of Promotion
• Physical & Immunizations Records (9th grade or more recent, out of state students must have IL physical)
• IEP/504/Special Ed Records (if applicable)
• Test Scores

Required for Transfer Students Documents Grades 10-12

Proof of Guardianship 

• Birth Certificate, Divorce/Custody/Guardianship Papers, /906 Placement Form
• ISBE Student Good Standing Form (Illinois transfer students)
• Withdrawal Papers
• Unofficial Transcript
• IEP/504/Special Ed Records (if applicable)
• Test Scores
• Physical & Immunizations Records (out of state students must have IL physical)
